Critical System Survey

Ask participants to name critical resources i.e. IT Systems (and some associated attributes), used by them, that are indispensable in the process of providing basic functionality expected by the public, and the rest of the local government from them or the area they perform their duties.

Survey: Identification of Critical Computer Systems

The purpose of this survey is to identify critical computer systems and data throughout the City. Please identify computer systems that are critical to the business of your department without which your department's ability to provide services to internal staff and or citizens would be seriously impaired during an emergency. Critical systems may include things as seemingly insignificant as a spreadsheet with emergency contacts, if your business unit's ability to supply services would be seriously impaired without that spreadsheet during an emergency.

Critical systems do not, however, include MS-Office products like Word, Excel, etc since we'll assume that we have to make those applications available during an emergency.
Please feel free to submit as many survey forms as needed to adequately cover your department's inventory of critical systems.
